What is a dental implant?

But what actually is a dental implant? This little device is basically made out of two parts - one that goes inside your body and the second that stays outside, in this case in your oral cavity.

The first part is an artificial "root", usually made out of medical titanium screw. This metal is extremely strong and has a very low chance of being rejected by your body. Only a very small portion of the population is allergic to this metal. To insert this part a periodontist makes a small cut in the gums and prepares the bone by making little holes in it. If the old tooth has been extracted or has fallen out recently, minimal bone changes have to be made. Doctor manipulates your bone very carefully and no damage is done to any structures or to osteoblasts - specific cells that bone tissue grows from. After preparing the bone the root part is firmly inserted. All this is done under local anaesthesia and is painless.

The bone regrows around the root securing it as if it was a part of your body. This usually takes from 2 to 6 months and then the dental crown is added. Sometimes a temporary crown is added before for an aesthetic appeal. If you are healthy the crowns can be added the same day as the root - with this special kind of implants you can have new teeth only after a few visits to a periodontist.

The artificial roots can also be used for supporting bridges and dentures, making you feel more comfortable about them. Usually, in these cases so called mini dental implants are used.

The implantation place will be painful and tender for a few days after the procedure. This will be due to a normal inflammation process because of the surgical treatment. Any tooth and any amount of teeth can be replaced with dental implants.

Before dental implant surgery

Before the surgery you will have to make a radiograph or a CT scan so your bone structures could be properly evaluated by a specialist. If necessary bone graft procedure might be considered. The doctor will also ask you about your chewing habits, bruxism (teeth grinding) and other conditions, such as osteoporosis. This will help to choose the optimal implant.

There are very little contraindications for the operation: uncontrolled type II diabetes, smoking or very bad oral hygiene.

After dental implant surgery

After the treatment you should take care of your new teeth as it were natural. Keep up with good home oral care and do not forget to visit your dentist or periodontist periodically. Although the teeth are now artificial but the bone, gum and other teeth are not.

Can something go wrong with the implantation?

The most common problem is the bone failing to integrate the implant. This can happen due to various causes, most often because of infection or limited amount of bone tissue around the implant. If this happens in a few weeks after implantation the implant can feel loose or the surrounding tissue becomes painful and irritated. This can result in the removal of the implant.

If the pain after the procedure is prolonged, the implant can also be considered for removal. Dental implants can be broken if in contact with direct force (for example blunt face trauma), just as real teeth. They can also fall out or loosen due to any other illness that would make your real teeth to do that. Except, of course, to caries, and you will never have to worry that a tooth can hurt because of a cavity in it.

The implant root is very well osteointegrated and a new tooth can feel different while chewing. But a lot of people forget that they have this kind of implants and maybe they will never remember it because implants can last for a lifetime. Dental implants are not only reliable, easy to take care of, aesthetically appealing, but also do not affect your other teeth.

Alternatives

You may also consider fixed conventional bridgework or removable partial denture. The advantages are that no surgery is needed but for bridgework usually healthy teeth have to be grounded down.

Partial denture is fast and economical but it does not stimulate the bone so it shrinks in time changing your facial features and making the denture not to fit properly which can negatively affect your speech.
